Attestation Form Follow-Up

The purpose of this follow-up is to measure the educational outcomes of this CME activity relevant to physician knowledge, competence, or performance.  This outcomes measurement is an integral part of successful CME and is in direct support of the criteria set forth by the Accreditation Council for Continuing Medical Education (www.accme.org).
